As per Institute Clause [ A ] All risk, extended to include risk of leakage, shortage and contamination, hijack, piracy, Including the risks of war clauses and strikes riots and civil commotions risks in terms of the relevant Institute Strikes Clauses in respect of all shipments and sending’s.
INFORMATION REQUIRED FOR ISSURANCE OF COVER/QUOTATION
- Estimated annual carrying - Total value of goods expected to be Imported/Exported in a a year (for annual cover)
- Limit any one carry/consignment - Value of goods in one shipment
- Description/nature of the consignment
- Bill of Lading
- Purchasing invoice/supplier invoice
- Packing list
CLAIMS PROCEDURES AND DOCUMENTS REQUIRED
- Notify the insurer or NMB claims team about the claim as soon as possible
- Copy of ID (NIDA, Driving License or Voters ID)
- Import declarations form
- Suppliers invoice
- Bills of lading
- Packing list
- Quantity Certificate
- Single bill of entry endorsed by customs
- Declaration and disposal order with attachments showing deliveries made
- Port Examination voucher for damaged items
- Clean report of findings
- Correspondences exchanged with the port operators or carriers and other parties regarding their liability for the loss or damage
- Survey report or other documentary evidence to show the extent of the loss or damage
- Any other information as may be requested by the insurer or NMB claims team
