• Estimated annual salaries for all staff

CLAIMS PROCEDURES AND DOCUMENTS REQUIRED

1. Notify the insurer or NMB claims team about the claim as soon as possible.
2. Copy of ID (NIDA, Driving License or Voters ID)
3. Duly completed claim form.
4. Salary slip for the preceding month before accident.
5. Medical bills (if any).
6. Medical report.
7. Police report PF 3 (in case of crime related accident).
8. Post mortem (in case of fatal injury).
9. Any other additional information as may be requested by insurer/NMB claims team.
